About the Role:
Join PERSONALIZED CARE FOR ALL LLC as a 245D Coordinator in St. Paul, MN, where you will play a pivotal role in enhancing the quality of life for individuals with disabilities. Responsibilities:

• Develop and implement individualized service plans in compliance with 245D regulations.
• Coordinate care services and resources for clients, ensuring seamless delivery of support.
• Conduct regular assessments and evaluations to monitor client progress and adjust plans as necessary.
• Facilitate communication between clients, families, and service providers to enhance collaboration.
• Maintain accurate documentation of client services and progress in accordance with state guidelines.
• Provide training and support to staff on 245D standards and best practices.
• Advocate for clients' rights and needs within the community and service systems.
• Stay updated on industry trends and regulatory changes to ensure compliance and quality service delivery.

Requirements:

• Bachelors degree in social work, human services, or a related field preferred.
• Minimum of 2 years of experience in coordinating services for individuals with disabilities.
• Knowledge of 245D regulations and best practices in service delivery.
• Strong organizational and communication skills, both verbal and written.
• Ability to work collaboratively with diverse teams and clients.
• Current Minnesota drivers license and reliable transportation.
• Compassionate demeanor with a commitment to client advocacy and empowerment.
• Certification in CPR and First Aid is a plus.
