Join our Stockton, CA team! We’re seeking a full-time Primary Care Physician with a competitive salary range of $315,900 – $386,600, offering the opportunity to deliver meaningful, patient-centered care to seniors in a community-based setting.

Requirements to Apply

• Completion of an accredited residency program (Internal Medicine, Family Medicine, Emergency Medicine, or Geriatrics)

• Active, unrestricted physician license with DEA registration and CDS certification

• Doctorate degree (MD or DO)

• Eligible for Medicare and Medi-Cal provider enrollment

Full-time Primary Care Physician role with a Monday–Friday schedule providing comprehensive, value-based care to senior patients in a community-focused setting.

Job Overview
As a Primary Care Physician, you will provide holistic, relationship-driven care to a smaller panel of senior patients, allowing you to make a meaningful impact on their health and quality of life. You will conduct comprehensive assessments, develop personalized care plans, and collaborate closely with an interdisciplinary team to ensure coordinated, patient-centered care. This role focuses on preventive and long-term care rather than high patient volume, with an average of eight patients per day and no hospital rounds. You will also support care transitions and participate in a rotating on-call schedule by phone.
