Requirements to Apply
Completion of an accredited residency program (Internal Medicine, Family Medicine, Emergency Medicine, or Geriatrics)
Active, unrestricted physician license with DEA registration and CDS certification
Doctorate degree (MD or DO)
Eligible for Medicare and Medi-Cal provider enrollment

Job Overview
As a Primary Care Physician, you will provide holistic, relationship-driven care to a smaller panel of senior patients, allowing you to make a meaningful impact on their health and quality of life. You will conduct comprehensive assessments, develop personalized care plans, and collaborate closely with an interdisciplinary team to ensure coordinated, patient-centered care. This role focuses on preventive and long-term care rather than high patient volume, with an average of eight patients per day and no hospital rounds. You will also support care transitions and participate in a rotating on-call schedule by phone.
