Responsibilities Include:

• Assessing patients' social, emotional, and financial needs
• Assisting with community resources and discharge planning
• Providing counseling and emotional support to patients and families
• Collaborating with physicians, nurses, and therapy staff to ensure quality patient care
• Advocating for patient well-being and independence

✅ Qualifications:

• Licensed Social Worker (LSW) required; MSW preferred
• Home health experience preferred, but not required
• Excellent communication and organizational skills
• Compassionate, patient-focused mindset
• Reliable transportation and valid driver's license
