Physician Internal Medicine-Geriatrics - Competitive Salary

A well-established outpatient primary care practice in Augusta, Georgia is seeking a Primary Care Physician to join its care team. This is a full-time outpatient primary care position serving an adult patient population. The practice emphasizes chronic disease management, preventive care, wellness visits, medication management, and continuity of care. Providers are supported by a team-based clinical structure designed to help physicians practice efficiently while spending meaningful time with patients.

Job Responsibilities

✅ Provide comprehensive outpatient primary care to adult patients
✅ Manage chronic conditions including diabetes, hypertension, COPD, CHF, and other common primary care needs
✅ Conduct wellness visits, follow-ups, preventive screenings, and medication reviews
✅ Collaborate with APPs, nurses, care managers, behavioral health, and other clinical support staff
✅ Support a care model focused on quality outcomes, access, prevention, and continuity of care
✅ Maintain accurate and timely documentation in the EMR

Qualifications

✅ Board-certified or board-eligible Family Medicine or Internal Medicine Physician
✅ Active Georgia medical license, or ability to obtain
✅ Primary care, internal medicine, family medicine, or geriatric care experience preferred
✅ Comfortable caring for adult and/or senior patient populations
✅ Strong communication skills and a team-oriented approach to care
✅ Commitment to preventive medicine, patient education, and long-term health outcomes
