Physician Family Practice-Geriatrics - Competitive Salary

A well-established outpatient primary care practice in York, Pennsylvania is seeking a Physician or Advanced Practice Provider to join its care team. This is a full-time outpatient primary care position serving an adult patient population, with an emphasis on preventive care, chronic disease management, medication management, and long-term continuity of care. The practice uses a team-based model designed to support providers while allowing meaningful time with patients.

Job Responsibilities

✅ Provide comprehensive outpatient primary care to adult patients
✅ Manage chronic conditions such as diabetes, hypertension, COPD, CHF, and other common primary care needs
✅ Conduct wellness visits, follow-ups, preventive screenings, and medication reviews
✅ Collaborate with physicians, APPs, nurses, care managers, and other clinical support staff
✅ Support a patient-centered model focused on quality outcomes and continuity of care
✅ Maintain accurate and timely documentation in the EMR

Qualifications

✅ Board-certified or board-eligible Family Medicine or Internal Medicine Physician, or licensed Nurse Practitioner / Physician Assistant
✅ Active Pennsylvania license, or ability to obtain
✅ Primary care, internal medicine, family medicine, urgent care, or geriatric care experience preferred
✅ Comfortable working with adult and/or senior patient populations
✅ Strong communication skills and a team-oriented approach to care
