RN Care Manager, Full-Time

Responsibilities:

• Maintain and improve the case management system using ongoing interaction with patients, physicians, and other health providers to meet designated clinical, operational, and financial outcomes for aggregate patient populations.
• Assist physicians, patients, families, staff, and payers in providing care for patient populations that are appropriate, effective, and cost-efficient through collaboration with payers to ensure proper utilization of patient care resources.
• Attend and actively participate in all departmental and interdepartmental meetings related to Case Management/Social Services/Discharge Planning functions.
• Contact patients following ED visits and hospital discharge to ensure understanding of their care, completing necessary documentation for Transition of Care, and arrange follow up visits with the PCP as needed. Ensure the patients receive timely recommended preventative services.
• Review patient medications to assess potential interactions and patient medication self-management.
• Assist patients with resources such as housing, transportation, food services, and other social services as appropriate to coordinate care with home- and community-based service providers.

• Ensure that patients receive comprehensive care.
• Document clearly and concisely all contacts and information of the patient’s case management process appropriately. Ability to develop a comprehensive care plan and document that Care Plan in a clear and concise manner. Complete billing for Care Management services.
• Establish, Educate, and maintain excellent working relationships with physicians, managers, and staff with a focus on building strong teams, competent and caring staff, improving processes, clinical excellence, and a customer-oriented environment.
• Assist in performing Medicare Annual Wellness visits.

Some of the AWV’s will be completed by PCPs, some by the RN Care Manager; the RN Care Manager will help coordinate these based on lists of patients that are eligible for these exams.

Requirements:

• Bachelor’s Degree in Nursing Preferred
• Three years of clinical experience and at least two years of case management experience are preferred or equivalent combination of education and experience.
• Certification in case management (preferred) or obtained within 3 years
• Current unrestricted Registered Nurse license in the State of Colorado (required)
• Current BLS required

Compensation:

• $39.00- $43.70 per hour, depending on education and experience.
• Discretionary bonuses, relocation expenses, merit increase, market adjustments, recognition bonuses, and other forms of discretionary compensation may be available.

• Relocation: Must relocate to Grand Junction, CO 81505 after starting work.
