Cuready Healthcare Staffing is seeking a qualified RN Case Manager to support patient care and clinical operations within healthcare facilities. The ideal candidate will deliver safe, high-quality care while collaborating with interdisciplinary care teams and maintaining compliance with facility policies and professional standards.

\n\nResponsibilities\n

• Coordinate patient care across the continuum, including planning, assessment, and evaluation.
\n
• Ensure accurate and timely case management documentation.
\n
• Manage patient discharges effectively by collaborating with healthcare providers.
\n
• Utilize evidence-based guidelines to determine medical necessity.
\n
• Perform needs assessments and coordinate necessary equipment orders.
\n
• Participate in care plan development and team meetings as required.
\n

\n\nQualifications\n

\n
• Current AK RN license is required.
\n
• Basic Life Support (BLS) certification preferred.
\n
• Minimum of 1 year of RN experience in an acute care setting.
\n
• Experience with Epic electronic health records system.
\n
• Preferred certification in ACM and experience in care coordination.
\n
• Willingness to work weekends as part of the regular schedule.
\n
