Job Details

Physical Therapist (PT) position at Above & Beyond Home Health and Hospice, part of Trinity Health system in Monticello, Iowa.
Job duration is 12 weeks with 40 hours per week.
Shift options are 8 AM to 4 PM or 9 AM to 5 PM.
Weekend work is as needed.
Epic electronic health record system experience preferred but not required.

Job Requirements

Must have a valid state license for Physical Therapy in Iowa, verified within 30 days of start.
American Heart Association BLS certification required for all direct patient care personnel.
Specialty specific competency testing must be completed within a year and attached.
Fit test, drug screening, and various health vaccinations and tests required (e.g., COVID-19 vaccine, MMR, Hepatitis B, Influenza vaccine, 2 Step PPD or IGRA, Varicella, Tdap).
Completion of Trinity Health professional forms and background checks including code of conduct, confidentiality agreement, integrity and audit services form, conflict of interest disclosure, criminal background check, education verification, EPLS, GSA/OIG, Joint Commission testing, NSO, system access form, skills checklist, child and adult dependent abuse training, Iowa sing report, and OFAC screening.
Physical/health statement documenting fitness to work within the last year.
