Licensed Practical Nurse - Long-Term Care, traveling assignment in Claremont, New Hampshire, United States. Begin on 06/30/2026 for a 13-week contract. Earn $603-$620 per week, with no guaranteed hours. Night shift, 16-hour shifts from 3:00 PM to 7:00 AM. An LTC - Nursing Home setting. This opening requires travel-status employment and a willingness to float to units with experienced staff.

Responsibilities
- Observe residents continuously for changes in mental and physical well-being; report pertinent findings to the Supervisor and provide a detailed handoff to the on-coming shift.
- Receive, transcribe, and implement physician orders; administer medications and treatments and document via MARs/TARs and electronic systems; manage unit pharmaceutical supplies.
- Supervise Licensed Nursing Assistants (LNAs) in alignment with NH Board of Nursing requirements; assign care within scope and provide direct support as time allows.
- Accompany physicians on rounds on assigned units; address medical needs and facilitate communication during 60-day reviews; assist with care planning and documentation.
- Administer oxygen as directed; perform gastrostomy/jejunostomy feedings per policy; conduct wound care and manage wound vacs; uphold infection control practices.
- Admit, discharge, and transfer residents; complete required documentation and 24-hour reports; maintain unit operations, track therapy appointments, and communicate with the Nurse Manager/Nursing Supervisor about issues.
Requirements
- Active Licensed Practical Nurse license in New Hampshire
- Minimum 3+ years of current nursing experience (within the last 6 months) – required
- Willingness to float to units where experienced staff are available – required
- IV certification (willing to obtain during onboarding if not certified) – required
- Covid-19 vaccination required; declinations/exemptions not accepted
- Local candidates within 50 miles of the assignment location will not be accepted; ensure permanent address is in profile
- Prior travel nursing experience and rural/small facility experience highly preferred
- BLS (AHA) certification; ability to work in a hospital-adjacent, long-term care setting
- Must have ongoing active licenses and be able to provide required certifications and references
- Documentation of 2 supervisory references from the last 2 years; resume that demonstrates current skills
