Job Summary

LPN Day and Night Correction Facility

Responsibilities

  • Provide licensed practical nursing care to day and night facility residents.

  • Administer medications and treatments per physician orders and facility policy.

  • Assess residents’ health status and update medical records accurately.

  • Monitor and respond to changes in resident conditions promptly.

  • Document care and outcomes in electronic health records with accuracy.

  • Uphold safety, infection control, and compliance with facility policies.

  • Collaborate with interdisciplinary team to develop and adjust care plans.

  • Engage in continuous learning to advance clinical skills and certifications.

  • Demonstrate accountability for own performance and contribute to team targets.

  • Participate in daily rounds and handoffs to ensure continuity of care.

  • Support discharge planning and resident/family education.

  • Maintain PPE use and environmental cleanliness to ensure high visibility of work.


Experience/Qualifications

  • Valid LPN/Licensed Vocational Nurse license in state of practice.

  • Proven experience administering medications and treatments per orders.

  • Strong documentation skills in electronic health records with accuracy.

  • Ability to assess changes in residents’ health and respond promptly.

  • Experience in long-term care or correctional facility settings preferred.

  • Infection control and PPE best practices adherence.

  • Collaborative care with interdisciplinary team and care plan updates.

  • Strong attention to safety, compliance, and facility policies.

  • Commitment to continuous learning and professional development.

  • Willingness to participate in daily rounds, handoffs, and discharge planning.
