Description
American Traveler is seeking an experienced RN for a Home Health position in the Greensboro/High Point, NC area, requiring prior case management experience and HCHB proficiency.
Details
- Community-based home health setting — not an inpatient position
- Covers the Greensboro and High Point service areas
- Role involves case managing an active patient caseload
- Monday–Friday, day shift schedule (8:00 AM – 5:00 PM)
- 13-week contract
Requirements
- Active NC RN license or compact RN license currently in hand
- Minimum 1 year of case management experience required
- Homecare Homebase (HCHB) EMR experience required
- Prior home health travel experience required
- Must have a personal vehicle for daily travel throughout the service area
- Two professional references required for consideration
- PDF copies of driver's license and Social Security card required for consideration
Additional Information
- Responsible for managing and coordinating care for an assigned home health patient caseload
- Will travel independently between patient homes throughout the Greensboro and High Point areas throughout the workday
- Mileage reimbursement provided at the federal rate
