Home Health Physical Therapist (PT) – Seaside, OR & Surrounding Areas
Assignment Details
- Shift: 8-Hour Day Shifts
- Facility cannot accommodate 4x10s or 3x12s schedules
- Patient Ratio: 1:1 patient care
- May visit up to 6 patients per day
- Standard productivity expectation: 30 productivity points/day
Experience Requirements
- Minimum 1 year of Skilled Home Health experience required
- First-time travelers accepted
Certifications & Licensing
- BCLS Certification required
- Pending license accepted
- Oasis experience required
Schedule & Call Requirements
- Weekend Requirement: None
- On-Call Requirement: None
- Floating Between Units: Not required
- Block Scheduling: Not available
Traveler Requirements
- Must have reliable personal transportation
- Daily travel radius may extend up to approximately 60 miles
- Estimated total drive time: 60–90 minutes/day
- Facility groups patient visits to minimize travel time
Vaccination Requirement
- Facility requires COVID-19 primary vaccination series plus one booster
