American Traveler is seeking a compassionate and experienced RN for a community hospice Case Manager position requiring a minimum of 2 years of experience and an active CO or compact RN license.

- Community hospice setting, providing care wherever patients call home including private residences, nursing homes, group homes, and assisted living facilities \n
- Primary RN role with an assigned patient caseload \n
- Monday through Friday, 8:00 AM – 5:00 PM schedule \n
- Occasional backup for weekends and on-call coverage required \n
- 6-week contract \n

- Active CO or compact RN license currently in hand \n
- Minimum 2 years of nursing experience required \n
- Strong clinical assessment and critical thinking skills \n
- Experience in symptom management and care for patients with varied diagnoses preferred \n

- Leads an interdisciplinary care team for an assigned patient caseload \n
- Serves as a patient and family educator, helping patients define personal goals of care and empowering caregivers to confidently support their loved ones \n
- Functions as a generalist across disease types with a specialization in end-of-life symptom management \n
