American Traveler is seeking an experienced RN for a Care Manager/Case Management position in an acute care hospital setting, requiring an active NC or compact RN license and at least 2 years of RN experience.

- Day shift schedule, 8:00am–5:00pm, 3 days per week (12-hour shifts) \n
- 36 hours per week \n
- Hospital-based Care Management unit within an academic medical center \n
- Covers a caseload across multiple specialty service lines including Cardiology, Nephrology, Trauma Surgery, Orthopedic Surgery, Vascular Surgery, Neurosurgery, GI Surgery, Urology, Transplant, and ICU/Stepdown \n
- Patient population consists of acutely ill individuals with complex and varying medical needs \n
- No on-call, weekend, or holiday requirements \n
- Epic EMR used for assessment and care plan documentation \n

- Active RN license in the state of North Carolina (NC or compact license) required \n
- Minimum of 2 years of RN experience required \n
- Hospital (acute care) case management, care management, or discharge planning experience required \n
- Nursing Diploma or ADN/ASN from an accredited nursing program required; BSN or MSN preferred \n
- Strong assessment and critical thinking skills required \n
- Epic EMR experience preferred \n
- Experience in academic or teaching hospital settings is highly beneficial \n
- Two professional references required for consideration \n

- Responsibilities include comprehensive patient assessment, care planning, and discharge planning to promote continuity of care and cost-effectiveness \n
- Participates in daily Care Management Touchpoints and weekly Complex Care Meetings (CCM) to monitor patient progression and address barriers to discharge \n
- Collaborates closely with an interdisciplinary team including Utilization Managers, Social Workers, and Care Management Assistants \n
- Coordinates family meetings, identifies post-acute service needs, and facilitates referrals to community resources and post-discharge providers \n
- Monitors observation patients throughout the day to ensure appropriate care progression \n
- Experience with care coordination and communication within interdisciplinary teams is strongly valued \n
