Description
In this role, you will manage patient care in the Case Management unit, ensuring smooth transitions for patients in a busy hospital environment. Working Monday through Friday from 8 AM to 4:30 PM, you will collaborate with a multidisciplinary team to provide high-quality care and support for patients and their families.
