American Traveler is seeking an experienced RN for a home-based Hospice and Palliative Care position in New Jersey, requiring a minimum of 2 years of home hospice RN experience and a valid NJ driver's license.

- Home-based hospice and palliative care setting serving patients in their place of residence \n
- Day shift schedule, 8am–4pm, 5 days per week (37.5 hrs/week) \n
- 16-week contract \n
- One weekend of on-call coverage required per month \n
- Floating to different counties required based on patient need \n
- EMR: Epic (experience required) \n

- Active NJ RN license (NJ or Compact) required \n
- Current BLS certification required \n
- Minimum 2 years of RN experience in a home hospice environment required \n
- Valid New Jersey driver's license required \n
- Associate's degree (ASN) or RN Diploma required \n
- Proficiency with EMR systems required; Epic experience preferred \n
- Strong critical thinking and communication skills required \n
- 2 professional references required for consideration \n

- Conducts patient assessments to evaluate appropriateness for hospice care and develops individualized plans of care for patients and families \n
- Manages a caseload as primary case manager, including developing, implementing, and evaluating evidence-based care plans \n
- Performs laptop data transfers twice daily and completes admission assessments and documentation within required timeframes \n
- Attends interdisciplinary team (IDT) meetings every two weeks and collaborates with the full care team to communicate patient progress \n
- Responsible for medication reconciliation, safe medication management, and maintaining current medication orders \n
- Provides patient and family education using age-appropriate teaching methods and documents all education tools used \n
- Participates in performance improvement activities, UR meetings, and provides mentorship and orientation to new staff as requested \n
- Candidates must reside more than 50 miles from the facility — local candidates are not eligible for this position \n
- Travelers may float to a nearby sister facility as needed \n
