Facility: Sylva Valley Health & Rehab
Job Type: Full-Time, Salaried
Schedule: Monday–Friday during active referral hours, with remote evening and weekend referral coverage as needed
Reports To: Administrator
Sylva Valley Health & Rehab is seeking a motivated, responsive, and relationship-driven Clinical Relations & Marketing Specialist (RN/LPN) to lead referral review, admissions coordination, and referral-source development.
This is a dedicated clinical liaison and admissions position with no routine floor-nursing assignments. The primary focus is responding quickly to referrals, making appropriate clinical admission decisions, maintaining consistent follow-up, developing strong referral relationships, and supporting facility census growth.
The successful candidate will be present at the facility each weekday while also conducting hospital visits, bedside evaluations, facility tours, and community outreach. Remote evening and weekend referral coverage will be required as needed.
Essential Responsibilities- Review incoming referrals and provide an initial response within approximately 15–30 minutes.
- Independently approve routine admissions that meet established facility guidelines.
- Escalate unusual, high-risk, or questionable referrals to the Administrator or designated clinical leader.
- Obtain missing clinical records, medication lists, therapy notes, insurance information, authorizations, and other required documents.
- Follow every pending referral until the individual is admitted, denied, or formally closed.
- Document the reason for each denied, delayed, withdrawn, or lost referral.
- Communicate admission decisions and outstanding requirements to hospitals, referral partners, residents, and families.
- Serve as the primary contact for urgent evening and weekend referrals, generally handled remotely.
- Conduct bedside evaluations and meet with prospective residents and families when needed.
- Provide facility tours and explain available clinical, rehabilitation, and long-term care services.
- Coordinate smooth transitions from hospitals, homes, assisted living communities, and other healthcare settings.
- Assist with verifying insurance coverage, payer information, authorization requirements, and financial documentation.
- Maintain accurate referral, admission, follow-up, and outreach records.
- Provide regular referral updates to the Administrator, Director of Nursing, admissions team, and other facility leaders.
- Develop professional relationships with hospital case managers, discharge planners, physicians, providers, home health agencies, hospice organizations, and other referral partners.
- Visit hospitals and referral sources regularly to maintain relationships and identify admission opportunities.
- Confidently and professionally ask referral partners for appropriate referrals and admissions.
- Collaborate with nursing, therapy, social services, the business office, admissions, and administration.
- Identify referral trends, admission barriers, missed opportunities, and strategies to improve conversion rates.
- Remain accountable for response times, completed follow-up, referral conversion, admissions, and census growth.
- Current and unrestricted RN or LPN license.
- At least one year of experience working in a skilled nursing facility is required.
- Strong clinical judgment and the ability to independently review referral information.
- Ability to manage multiple active referrals without missing follow-up, documentation, or deadlines.
- Excellent communication, organization, customer-service, and relationship-building skills.
- Confident and persuasive when discussing facility services and requesting appropriate referrals.
- Comfortable managing urgent admission needs and working toward performance and census-growth goals.
- Dependable, self-directed, and capable of making timely decisions.
- Professional appearance and conduct.
- Reliable transportation for hospital visits, bedside evaluations, and referral-source outreach.
- Ability to be physically present at the facility each weekday.
- Ability to provide remote evening and weekend referral support when needed.
Previous experience in any of the following areas is preferred:
- Skilled nursing admissions
- Clinical liaison services
- Long-term or post-acute care
- Hospital discharge planning
- Healthcare marketing or business development
- Insurance verification and authorization
- Census development
Training will be provided on the facility’s referral platforms, hospital portals, PointClickCare, admissions procedures, and required technology.
