We are seeking experienced, dependable, and compassionate Senior Certified Nursing Assistants (CNAs) to join a dedicated private-duty care team supporting an adult patient with complex medical and rehabilitation needs in a private home environment.

This is not a traditional CNA role. This position is ideal for advanced CNA’s with a strong background in rehabilitation, experience supporting stroke or brain injury recovery, ICU support, or high-acuity patient care who are seeking a stable long-term opportunity in a respectful and team-oriented setting.

The ideal candidate is confident assisting with advanced mobility support, repositioning, transfers, tracheostomy support, feeding tube assistance, and comprehensive daily care needs while working closely with therapists, caregivers, and family members as part of a collaborative care team.

The home environment is calm, professional, and supportive, with a strong focus on patient safety, dignity, comfort, and quality of life. Caregivers are valued members of the team and are treated with appreciation, professionalism, and respect.

Shift Availability

Day Shift

7:00 AM – 7:00 PM

Night Shift

7:00 PM – 7:00 AM

  • Full-Time & Part-Time positions available
  • Rotating weekends and holidays may be required

Position Overview

The CNA will provide hands-on support and one-on-one care in a private residential setting. This role requires strong attention to detail, professionalism, adaptability, and the ability to work closely with therapists, caregivers, and family members in a coordinated care environment.

Candidates should be comfortable working with patients who require advanced mobility assistance, repositioning, tracheostomy support, feeding tube assistance, and comprehensive daily care needs.

Responsibilities

  • Provide direct one-on-one patient care
  • Assist with activities of daily living including, bathing, dressing, grooming and hygiene assistance
  • Support patient mobility, positioning, and transfer needs using safe body mechanics and lift equipment
  • Perform routine repositioning and pressure-relief measures to maintain skin integrity and patient comfort
  • Assist with transfers using Hoyer lifts, lateral transfer techniques, and other mobility equipment
  • Monitor and document vital signs including temperature, pulse, respirations, and blood pressure
  • Observe and promptly report changes in patient condition
  • Assist with rehabilitation exercises and therapy-related activities as directed by the care team
  • Maintain a safe, organized, and clean patient environment
  • Support feeding tube care and daily nutritional assistance
  • Assist with tracheostomy-related care and basic airway support as directed
  • Provide compassionate emotional support to the patient and family
  • Maintain professionalism, patient dignity, and privacy at all times
  • Most importantly- must be proactive, attentive and have a very high attention to detail

Preferred Experience

Strong preference will be given to candidates with experience in:

  • Neurological rehabilitation
  • Traumatic brain injury (TBI) recovery
  • Intensive care units (ICU)
  • Long-term acute care (LTAC)
  • Rehabilitation units
  • Private-duty or home healthcare settings

Qualifications

  • Active CNA certification in good standing
  • Minimum 5 years of CNA or caregiving experience preferred
  • Experience working with neurological or rehabilitation patients strongly preferred
  • Experience assisting patients with, Tracheostomy care and G-tube support
  • Transfers and repositioning
  • Mobility assistance
  • Knowledge of infection control and patient safety procedures
  • Strong communication and teamwork skills
  • Ability to work independently while maintaining professionalism
  • Ability to prioritize tasks in a fast-paced care environment
  • Ability to lift and assist with patient transfers of at least 50 lbs using proper lifting techniques
  • COVID-19 vaccination compliance required
  • Ability to pass a background check and provide professional references
