Job Description
Position Summary
The primary role of a Licensed Practical Nurse (LPN) is to provide comprehensive oversight, coordination, and hands-on care in addressing all health-related matters affecting program participants. This includes both direct and indirect health concerns, ensuring the physical and emotional well-being of individuals through proactive assessment, timely intervention, and ongoing collaboration with interdisciplinary team members, families, and external healthcare providers. The LPN plays a critical role in maintaining a safe, responsive, and supportive environment that promotes optimal health outcomes for all individuals involved in the program.
Major Responsibilities
- Maintains nursing documentation and medication records
- Assists residents in learning about their medications; assures that medications are taken as physician ordered and recorded in electronic MAR
- Provides nursing care including, but not limited to, wound care, trach care, diabetic counseling, etc
- Assesses residents’ health and emotional needs
- Communicates with other members of the nursing and program staff; communicates serious issues to Nurse Manager and Program Director
- Maintains confidentiality, including keeping voice low and not talking about residents in public areas
- Other duties as assigned
Job Qualifications
- Current PA RN/LPN licensure
• Current CPR/First Aid certification
• Valid Driver’s License
• Act 33/34 clearances
• FBI clearance as needed
• All trainings required by licensure
- Must complete a Physical, TB test, and Hep B titer.

- Patient Care: LPN’s assess patient health, and implement nursing interventions to ensure optimal patient outcomes.
- Medication Administration: Administer medications and monitor patients for side effects and reactions, ensuring adherence to prescribed treatment plans.
- Collaboration: Work closely with physicians and other healthcare professionals to coordinate patient care and communicate updates regarding patient conditions.
- Documentation: Maintain accurate and complete electronic patient records, documenting vital signs, symptoms, and changes in patient conditions.
- Patient Education: Educate patients about health conditions, treatment options, and self-care strategies to promote recovery and wellness through individual and group sessions.
- Proficient use of computers, mobile devices, and agency software.
- Business use of company equipment including vehicle.
