Pleasant Valley Nursing & Rehab Center is seeking a PRN Occupational Therapist (OT) to support residents with functional goals related to safety, independence, and daily activities.

Responsibilities:

• Complete occupational therapy evaluations and treatment sessions
• Address ADLs, upper extremity function, coordination, and adaptive strategies
• Collaborate with rehab staff and families regarding care plans
• Monitor resident progress and adjust treatment as needed
• Maintain compliant documentation

Qualifications:

• Active OT license required
• Skilled nursing experience preferred but not required
• Strong communication skills
