Pleasant Valley Nursing & Rehab Center is seeking a PRN Physical Therapist (PT) to join our rehabilitation team. This opportunity offers flexible scheduling while supporting residents with mobility, strength, and functional goals in a skilled nursing setting.

Responsibilities:

• Complete physical therapy evaluations and provide individualized treatment
• Address gait training, balance, strengthening, transfers, and endurance goals
• Collaborate with rehab staff and nursing on resident care plans
• Monitor progress and adjust treatment as needed
• Maintain timely documentation

Qualifications:

• Active PT license required
• Skilled nursing experience preferred but not required
• Strong clinical and communication skills
