Responsibilities
Provide direct patient care under RN supervision, documenting assessments and vitals.
Administer prescribed medications and treatments following facility policies.
Monitor patient status, report changes promptly to the care team.
Ensure compliance with safety, infection control, and privacy protocols.
Participate in skills development and continuing education to advance expertise.
Maintain accurate, timely documentation in patient records and care plans.
Collaborate within a team to meet performance targets and care quality metrics.
Utilize care pipelines and target-driven metrics to identify improvement opportunities.
Assist with activities of daily living to support resident independence.
Support discharge planning and coordinate with families and caregivers.
Maintain on-site presence and accessibility for residents and team during shifts.
Experience/Qualifications
LPN license and current nursing practice in long-term care settings.
Proven ability to administer medications and document treatments per policy.
Experience with vital signs, assessments, and care plan documentation.
Proactive monitoring of resident status with timely escalation of changes.
Strong teamwork and communication with RN, patients, families, and care staff.
Knowledge of safety, infection control, and privacy protocols.
Comfort with on-site presence, flexibility across shifts, and accessibility.
Participation in ongoing education and skills development.
Ability to support activities of daily living and resident independence.
Experience with discharge planning and caregiver communication.
