This setting is ideal for nurses who value a close-knit community feel, shorter commutes, and the ability to make a visible impact on the well-being of local residents.As a Home Health RN in this role, you will provide skilled nursing care to patients in their homes across Greenville and surrounding areas. Typical responsibilities include performing comprehensive assessments, developing and updating individualized care plans, administering medications and treatments, managing wounds, monitoring chronic conditions, and coordinating with physicians and other members of the healthcare team. A typical day may include a caseload of home visits scheduled within a defined geographic territory to support efficient travel and consistent patient follow-up.Your work will focus on helping patients remain safely in their homes, avoid unnecessary hospitalizations, and achieve their health goals through personalized, compassionate care. You will educate patients and caregivers on disease management, medications, and safety strategies, tailoring your approach to each home environment. Documentation is performed using an electronic medical record system, with point-of-care charting whenever possible to reduce paperwork and keep your time focused on patient care.Shifts are generally daytime hours with scheduling designed to support both patient needs and work-life balance. The role may include participation in an on-call rotation, weekend or holiday coverage as needed to ensure continuity of care. Patient acuity and visit volumes are balanced to promote safe and effective practice, and you will collaborate with a supportive interdisciplinary team that may include physical, occupational, and speech therapists, social workers, and other nursing professionals.This position offers the opportunity to develop strong clinical skills in the home health setting, gain experience with complex medical conditions managed outside the hospital, and build meaningful relationships with patients and families over time.
