About the OpportunityKARE CENTER in Ann Arbor, MI is seeking a dedicated and compassionate Nurse Practitioner (NP) to join our growing multidisciplinary healthcare team. In this role, you will provide comprehensive patient care including medical and psychiatric evaluations, diagnosis, treatment planning, medication management, and patient education. You will collaborate closely with physicians, psychiatrists, therapists, and other healthcare professionals to deliver high-quality, integrated, patient-centered care across a diverse patient population.
Key Responsibilities
  • Conduct comprehensive patient evaluations including history, physical examinations, mental status assessments, and risk evaluations.
  • Diagnose and manage a range of acute, chronic, and psychiatric conditions in collaboration with supervising physicians.
  • Develop and implement individualized treatment plans incorporating pharmacotherapy, referrals, and community resources.
  • Prescribe and manage medications, monitoring patient response and adjusting treatment plans as clinically indicated.
  • Provide patient education and psychoeducation to patients and families regarding diagnoses and treatment.
  • Conduct follow-up appointments to monitor patient progress, medication efficacy, and overall well-being.
  • Assess and manage urgent clinical situations, coordinating appropriate levels of care as needed.
  • Maintain accurate, thorough, and timely clinical documentation in the electronic health record (EHR).
  • Participate in interdisciplinary team meetings and quality improvement initiatives.
Qualifications & Requirements
  • Master of Science in Nursing (MSN) or Doctor of Nursing Practice (DNP) from an accredited program.
  • Active Advanced Practice Registered Nurse (APRN) license and NP certification in Michigan (or eligibility to obtain).
  • DEA registration (or eligibility to obtain) for controlled substance prescribing.
  • 1-3 years of clinical experience in primary care, behavioral health, or integrated care preferred.
  • Strong clinical assessment, diagnostic, and treatment planning skills.
  • Proficiency with electronic health record (EHR) systems.
  • Excellent interpersonal and communication skills with a compassionate, patient-centered approach.
