Compensation: $120,000 to $160,000+ Total Compensation (Base Salary + Productivity Incentive Program) + Mileage Reimbursement

As a Wound Care Nurse Practitioner, you'll step into a role where your clinical decisions have measurable results. You'll work directly with patients across assisted living, memory care, skilled nursing, and post-acute communities while helping improve healing rates, reduce avoidable complications, and strengthen care coordination.

What You'll Be Doing
• Provide advanced wound care assessments, treatment planning, and ongoing management of complex wounds.
• Conduct routine rounding visits throughout post-acute, assisted living, memory care, and long-term care communities.
• Manage pressure injuries, surgical wounds, diabetic ulcers, skin integrity concerns, and related conditions.
• Collaborate closely with physicians, nurses, therapists, and facility teams.
• Support chronic disease management, preventative care initiatives, and continuity of care.
• Help reduce avoidable hospital readmissions through proactive intervention and follow-up.
Qualifications
• Active Minnesota Certified Nurse Practitioner (CNP) license (AGNP preferred).
• Experience in wound care, post-acute care, geriatrics, primary care, skilled nursing, or related settings preferred.
• Strong clinical judgment and confidence managing medically complex patient populations.
• Valid driver's license and ability to travel throughout the Twin Cities Metro Area.
