Speech-Language Pathologist (SLP) SNF

Position Description:

We are seeking a dedicated and skilled Speech-Language Pathologist (SLP) to join our team in a skilled nursing facility (SNF). This role is essential for providing comprehensive evaluation and treatment for patients with speech, language, and swallowing disorders.

Job Details:

• Shift: Day, 3x8 Hours (09:00 - 17:00)
• Weekly Hours: 20 hours
• Daily Patient Load: 5-9 patients
• Orientation Hours: 8 hours
• Patient Age Group: Geriatrics

Essential Responsibilities:

• Perform assessments and evaluations for patients with various diagnoses.
• Develop, implement, and revise individualized treatment plans.
• Facilitate rehabilitation through specialized therapy techniques.
• Educate patients and families regarding treatment plans and progress.
• Document patient progress using electronic medical records, following SOAP note format as appropriate.

Qualifications:

• Minimum of 3 years of experience in a similar setting is required.
• Current state license and BLS certification are required.
• Experience with adult outpatient, adult rehabilitation, and skilled nursing settings is preferred.
• Special training or certification in working with patients with dementia is advantageous.
• Experience with various conditions, including but not limited to, CVA, dementia, dysphagia, and cognitive impairments is preferred.

Skills Required:

• Assessment, evaluation, and documentation skills in accordance with Medicare A and B.
• Knowledge of dysphagia management, including bedside swallow evaluation and compensatory strategies.
• Ability to utilize alternative communication devices and strategies for effective patient care.
• Experience with functional maintenance programs and in-service training.

Work Environment:

The SLP will be the sole therapist in a small therapy department within a 113-bed skilled nursing facility. The position includes collaboration with an interdisciplinary team, including physical therapy and social services. The facility has a diverse patient population with varying diagnoses and treatment needs.

Flexibility:

This position requires flexibility with scheduling, including shared responsibilities between two facilities. The ideal candidate should be willing to travel between locations as necessary.
