About this job

Looking for a change of scenery and a chance to help others. Provide skilled physical therapy services to home health patients in Demopolis, AL while working in a community-based setting. Perform patient evaluations and deliver therapeutic interventions according to physician orders. Supervise and evaluate physical therapy assistants and home health aides as assigned. Maintain documentation, communicate with physicians and interdisciplinary team members, and evaluate the home environment to recommend appropriate safety or treatment modifications. Reliable transportation, valid driver’s license, and agency-required liability insurance may be required.


Minimum Requirements

  • Bachelor’s, Master’s, or Doctoral degree in Physical Therapy from an accredited program approved by relevant accrediting bodies.
  • Current, unencumbered Alabama physical therapist license required prior to the start of the assignment.
  • Current CPR/BLS certification from American Heart Association or American Red Cross.
  • Experience providing physical therapy in home health or community-based settings preferred.
  • Ability to supervise PTAs and home health aides, including evaluation and delegation of treatment plans.
  • Familiarity with documentation systems and basic computer skills, including email and electronic charting.
  • Reliable transportation and valid driver’s license required for home visits. Agency-required liability insurance as applicable.
  • Two to three years of physical therapy experience preferred where noted by state-specific requirements.


Preferred Qualifications

  • Previous travel or per diem home health experience.
  • Experience working with interdisciplinary home health teams and coordinating discharge plans.
  • Experience supervising PTAs with knowledge of appropriate supervision intervals and documentation practices.


Assignment Details

  • Shift: Evenings, 5x8
  • May require travel between patient homes within the service area and occasional floating to other service locations as needed.
