Minimum Requirements

  • Bachelor's, Master's, or Doctoral degree in Physical Therapy from an accredited program approved by the APTA.
  • Active Arkansas State Physical Therapy license required to start the assignment.
  • Home Health and OASIS experience required.
  • CPR certification required.
  • Must be able to complete patient evaluations, develop PT plans of care, and document progress per state and agency policy.
  • Ability to visit patients across the agency service area and nearby agencies as needed. Mileage reimbursed between visits from first patient to last patient.
  • Schedule flexibility required to participate in weekend rotation and potential holiday availability as needed.


Preferred Qualifications

  • Experience with HCHB electronic health record preferred.
  • Previous supervision of PTAs or home health aides preferred when allowed by state regulation.


Assignment Details

  • Shift hours: 2000–0400.
  • Weekly hour guarantee: 32 hours per week.
  • On call/weekend requirement: no (note schedule flexibility and participation in weekend rotation may be requested).
  • Coverage area includes Eureka Springs, AR area; must be able to travel within service area.
  • Dress code scrub color: Royal Blue.

Impacting Patient Care

Provide in-home physical therapy services that evaluate functional status, develop and implement individualized plans of care, and monitor patient response to treatment. Coordinate with physicians, nursing, and caregivers to report changes in condition, supervise PTAs and aides as allowed, and complete regular documentation including progress notes and discharge summaries. Adhere to state regulations and agency therapy standards of practice. Maintain required licensure and pursue continuing education to stay current with standards of care.
