Minimum Requirements

  • Bachelor's, Master's, or Doctoral degree in Physical Therapy from an accredited program approved by the APTA.
  • Active Alabama Physical Therapist license required to start the assignment.
  • BLS certification from AHA or ARC may be required.
  • Home Health experience required.
  • Experience performing comprehensive PT evaluations, establishing measurable goals, and developing/modifying treatment plans in accordance with physician orders and regulatory requirements.
  • Experience supervising Physical Therapist Assistants and aides per state practice act.
  • Documentation skills to complete evaluation, treatment, and billing records per company, facility, and payer timelines and formats.
  • Ability to consult with physicians and interdisciplinary team regarding patient progress and discharge planning.
  • Preferred: Experience with HCHB electronic documentation system.

  • Facility type: Home Health.
  • Shift name: Days.
  • Shift details: Monday–Friday, 0800–1700.
  • Productivity expectation: 30 points.
  • Counties covered: Butler, Crenshaw, Lowndes, Covington.
  • Floating requirement: Clinician may be required to travel within the county coverage area for patient visits.
  • Required license: Physical Therapist (state license active).
