Nurse Practitioner

Greater Shores Primary Care is looking for a dedicated Nurse Practitioner (NP) to join our team in providing high-quality healthcare to our patients. Working closely with Dr. Amine, the Nurse Practitioner will play an essential role in diagnosing, treating, and managing patient health while fostering a caring, patient-centered environment. We are seeking a skilled professional who is committed to enhancing patient outcomes and building strong relationships within the community.

Key Responsibilities:

• Perform comprehensive patient assessments, including health histories and physical examinations
• Diagnose and manage acute and chronic conditions using evidence-based practice
• Collaborate with Dr. Amine and the healthcare team to develop individualized care plans
• Prescribe medications and treatments as appropriate, following state regulations
• Educate patients and families on wellness, preventive care, and chronic disease management
• Provide routine follow-up care and manage patient health records efficiently
• Participate in quality improvement initiatives and ensure adherence to clinical best practices
• Deliver compassionate care with clear and effective communication

Qualifications:

• Active and unrestricted Nurse Practitioner license
• Board certification as a Family Nurse Practitioner (FNP) or Adult-Gerontology Primary Care Nurse Practitioner (AGPCNP)
• Minimum of 2 years of experience in a primary care or outpatient setting (preferred)
• Strong clinical judgment and ability to work independently and as part of a team
• Excellent communication and interpersonal skills
• Proficient with electronic health records and telehealth services
• Passion for patient education and a holistic approach to healthcare
