Ambulatory Care Pharmacist

Ambulatory Care Pharmacist job at busy outpatient clinic in Pennsylvania.

Monday - Friday, day-shift position 

 Am Care duties: 

• Direct care of patients
• Education of physicians and staff regarding clinical information
• Implementation of clinical programs that involve patients/ drug therapy. 
• Responsible for managing the selection and utilization of pharmaceuticals and supports core clinical programs such as DUR, DIS and formulary management.

Ideal candidate: BS in Pharmacy or PharmD.  Prior Ambulatory Care experience or PGYI.  Active PA pharmacist license or ability to obtain.
