Responsibilities
Assess patient needs and develop targeted treatment plans.
Deliver evidence-based physical therapy interventions for optimal recovery.
Document sessions and track patient progress in the EMR system.
Collaborate with multidisciplinary teams to coordinate care.
Set and monitor personal performance metrics and patient outcomes.
Identify patients suitable for referrals and participate in pipeline growth.
Demonstrate accountability for own outcomes within a results-driven environment.
Educate patients and families on exercises and self-management strategies.
Maintain adherence to safety, hygiene, and clinic protocols.
Engage in ongoing professional development and attend relevant trainings.
Provide timely progress updates to supervisors and document milestones.
Support clinic-wide initiatives to improve patient access and throughput.
Experience/Qualifications
Licensed Physical Therapist with active state license
Assess patients and design targeted, evidence-based treatment plans
Deliver PT interventions and document progress in EMR accurately
Demonstrate accountability for outcomes in a results-driven environment
Collaborate with multidisciplinary teams to coordinate care
