Position Overview

HumanEdge Health is seeking a Travel Physical Therapist for an exciting assignment in John Day, OR. This unique opportunity combines primarily home-based outpatient care (90%) with a small inpatient component (10%), allowing you to build meaningful patient relationships while providing high-quality rehabilitative care across settings.

This role is ideal for a PT who enjoys working independently, delivering personalized care, and making a direct impact on patients' functional outcomes within their homes and community.

Key Responsibilities

• Evaluate and treat patients in home-based outpatient and inpatient settings
• Develop and implement individualized treatment plans
• Provide evidence-based interventions to improve mobility, strength, balance, and overall function
• Educate patients and caregivers on treatment plans and home exercise programs
• Maintain accurate and timely clinical documentation
• Collaborate with physicians and interdisciplinary team members to optimize patient outcomes

Requirements

• Current Oregon Physical Therapy license (or ability to obtain)
• Graduate of an accredited Physical Therapy program
• Previous outpatient and/or home health experience preferred
• Strong clinical, communication, and organizational skills
• Ability to work independently and manage a varied caseload
