Status: Full-time, hourly, non-exempt. 32-40 hours per week.
Schedule: Tuesday-Saturday, 8:00 AM to 5:00 PM. Work outside of these hours may be required.
Location: Working all throughout Central Oregon in patients' homes and facilities.
Pay: $22.00-30.00 per hour, based on experience.

Essential Functions:
- Communicates in a professional and caring manner to the patient, family, and/or caregivers while maintaining confidentiality of patient and related health information.
- Alerts nursing staff if there is a change in the patients behavior or condition or the patient requires assistance or other nursing intervention outside the scope of practice.
- Schedules initial visit in a timely manner after order received from care manager and plan of care developed. Schedules repeat visits in conjunction with patient, family, facility and adheres to ordered visit frequency.
- Assesses the patients ability to tolerate treatment before beginning care. Assesses patient condition after care.
- Provides personal care as outlined on the plan of care, maintaining patient privacy while demonstrating sensitivity to patient comfort and privacy.
- Meets the safety needs of the patient and self by utilizing good body mechanics and proper use of equipment. Provides extension of therapy services by assistance in ambulation or exercise as delegated by the therapist and outlined in the plan of care.
- Coordinates with care manager to ensure plan of care is updated and accurate and reports any changes in patients condition. Documents all patient care with proficiency in compliance with policies, procedures and regulatory agencies.
- Obtains and documents patient vital signs and communicates changes/concerns as outlined on the plan of care to the care manager. Assesses patient pain utilizing organization pain scales and reports to care manager per plan of care. Observes patients skin and provides appropriate care to prevent or mitigate breakdown of tissue.
- Performs certain functions when delegated by the registered nurse as outlined by the State Board of Nursing delegation rules including assisting with other tasks and duties as outlined on plan of care and scope of practice.
- Participates in educational programs and in-service meetings as requested. Completes at least 12 hours continuing education per calendar year related to area of specialization to remain current in the field.
- Complies with all applicable laws, regulations, policies and procedures.
- Adheres to the PIC mission and values. Supports the organization.
- May be exposed to infections and contagious diseases. Contact with patients under wide variety of circumstances.
- May be exposed to/occasionally exposed to patient elements. Subject to varying and unpredictable situations.
- Handles emergency or crisis situations.
- OSHA exposure category 1: Position includes tasks that involve exposure to blood, body fluids, tissues.
- Required Personal Protective Equipment as conditions warrant: Gloves, gown, mask, mask with shield, goggles, shoe covers, bouffant cap.
- Unencumbered Oregon CNA license.
- Ability to pass a reference check, background test, drug screen, and TB test.
- Graduate of an accredited CNA program.
- At least 18 years of age.
- One to two years of recent medical care experience in an institutional setting. Ideal candidates will possess 1-2 years of home health or hospice experience (preferred).
- Valid CPR Certification.
- Current automobile insurance, a valid Oregon drivers license, and reliable transportation.
- Strong communication and interpersonal skills.
- Ability to work independently and collaboratively within an interdisciplinary team.
