Service Coordinator

• Vermont Comforts of Home (VCOH) offers adults a choice to remain in the community when they are no longer able to live on their own and also require a nursing home level of care.
• VCOH is a person-centered program.
• Service Coordinators meet at least monthly with the shared living provider and participant in their home to support and manage various aspects of the state-required expectations to ensure a healthy environment that meets the participants goals and needs.
• Mix of field work, work from home and main office in Essex Junction at least three days per week.
• Background checks are required
• Must have valid drivers license and insured vehicle
• Generous benefits, holiday schedule and compensation package
• Thorough initial and ongoing training and support for Service Coordinators
• Mileage reimbursement provided for field work visits
• M-F 8am-4pm
• College degree in Human Services or related field preferred
