87 yr old woman, after hospitalization for hyponatremia (low sodium) she has confusion and fatigue so she needs someone to visit once a day around 12:00 p.m. on Wednesdays and Sundays. She would need help with taking her blood pressure, remembering her meds, to occasionally do shopping, drive her to Dr.'s appointments and events and perhaps light housekeeping.
