Hours: 30–38 hours per week
Schedule: Rotating including weekends and possible holidays

Position Overview

Family Nurse Practitioner (FNP) to join our clinical team in a float capacity supporting clinic locations across Gainesville and Macclenny, Florida.

This provider will work collaboratively with a dedicated clinical care team to deliver evidence-based primary and family care services focused on improving patient outcomes through assessment, diagnosis, treatment planning, chronic disease management, wellness services, and patient education.

The Nurse Practitioner will report directly to the Practice Manager and practice with a high level of autonomy while delivering exceptional patient-centered care.

Schedule Requirements

Clinic Hours of Operation:

Monday – Friday: 8:30 AM – 7:30 PM
(1-hour lunch)

Saturday: 9:00 AM – 5:30 PM

Sunday: 9:00 AM – 4:30 PM
(30-minute lunch)

Required Availability:

  • 2–3 weekdays per week
  • 6 of every 12 weekend shifts (Saturday/Sunday rotation)
  • Holiday rotation participation required

Key Responsibilities Clinical Care Delivery

  • Perform evidence-based primary and family care assessments
  • Diagnose and treat acute and chronic health conditions
  • Develop individualized treatment and care plans
  • Provide preventive health counseling and wellness education
  • Conduct chronic disease management interventions
  • Perform routine physical examinations and screenings
  • Order, interpret, and review diagnostic testing
  • Coordinate care plans with interdisciplinary healthcare professionals
  • Promote patient engagement and self-management strategies

Patient Management

  • Manage scheduled appointments efficiently
  • Ensure positive patient health outcomes through follow-up care
  • Provide health screenings and behavioral health counseling
  • Support continuity of care planning and referrals as needed

Administrative & Clinic Operations

  • Open and close clinic as assigned
  • Verify insurance and collect payments
  • Maintain clinical inventory and supplies
  • Ensure clinic cleanliness and compliance standards
  • Complete documentation accurately in EMR systems

Required QualificationsLicensure & Certification

  • Active RN License in Florida
  • Active APRN License in Florida
  • Current National Board Certification as Family Nurse Practitioner
  • Current BLS Certification through:
  • American Heart Association
  • American Red Cross
  • DEA registration (or willingness to obtain during onboarding)

Education

Master’s Degree from an accredited Family Nurse Practitioner program

Clinical Experience

Minimum 1 year recent Nurse Practitioner experience in one of the following:

  • Primary Care
  • Family Medicine
  • Urgent Care

Preferred Skills

  • Strong chronic disease management experience
  • Knowledge of quality metrics and evidence-based care standards
  • Experience with Electronic Medical Systems (EMR/EMS)
  • Strong multitasking and organizational skills
  • Autonomous clinical decision-making
  • Adaptability and flexibility across clinic locations
  • Excellent written, verbal, and electronic communication
  • Technology proficiency
  • Willingness to obtain multi-state licensure
