Job Description

As a Home Health Speech Language Pathologist with occasional travel, you will deliver specialized speech therapy services primarily to pediatric patients in both clinic and home health settings. Managing a caseload of 11-20 patients, you will evaluate, plan, and provide individualized therapy focused on articulation, language, and cognitive-communication disorders. Your role involves working independently while occasionally using telepractice platforms and assistive communication devices to enhance patient care.

 

Responsibilities

  • Conduct comprehensive patient assessments for speech and language disorders
  • Develop and implement individualized, goal-driven treatment plans
  • Provide therapy sessions addressing articulation, language delays, and cognitive-communication impairments
  • Monitor and document patient progress consistently
  • Educate families on support strategies and therapeutic techniques
  • Collaborate with multidisciplinary team members when applicable
  • Utilize telepractice platforms and assistive communication devices to support therapy delivery
  • Manage thorough case documentation and follow-up procedures
  • Travel occasionally to patient homes to ensure comprehensive care

 

Required Qualifications

  • Minimum 1 year experience in speech-language pathology
  • Master's degree in Speech-Language Pathology
  • State licensure as a Speech-Language Pathologist
  • Certificate of Clinical Competence in Speech-Language Pathology (CCC-SLP)

  • Proficiency in speech-language evaluation and treatment planning
  • Experience in pediatric speech therapy
  • Strong patient communication and family education skills
  • Accurate, thorough documentation practices
