Description
Job Summary
TheLicensed Practical Nurse actively uses the nursing process inrenderingtotal patient care under supervision of an RN andin accordance withthe nursing care plan and medical plan of care. Provides direct patient care, meeting both psychological and physical needsin accordance withthe plan of care,physician'sorders, and nursing and department policies and procedures.
Responsibilities And Scope
Contributes to the development and maintenance of a current, individualized, goal-oriented plan of care and consults with other health professionals toassurethat the assessed needs of patient/caregiver are met.
Observes, assesses, documents, andreportspatient response to treatments/therapies and teaching, and adjusts priorities of care according to patient/caregiver needs. Collects, documents, and reports dataregardingpatient's biophysical, psychosocial, environmental, self-care, educational, and discharge planning needs.
Participates in unit, service, and/or department enrichment activities which contribute to positive outcomes such as assigned committees.
Observes, collects, and documents patient information obtained while providing care and promptly report significant changes in the patient's condition. Demonstrates proper performance of skills reflected andvalidatedby the position's orientation/skills checklist.Usesstandard precautions and principles of infection control.
Provides assistance topatients and families during admission, orientation to the hospital experience, and discharge.Provides support to patients and families.Verbally de-escalates frightened or potentially aggressive patients. Facilitatesage-appropriatecoping strategies for patients.
Qualifications
Required
Graduate of an accredited school of practical nursing
Current LPN licensure in the state of North Carolina
Basic Life Support (BLS) certification
