Location: Portland, Oregon - 97213
Schedule: 5×8 Hour Days | 8:00 AM – 4:30 PM
Contract Length: 13 Weeks with possible extension
Start Date: July 13, 2026 

*Requires both an active OR and WA license*

 Position Overview

Seeking an experienced Registered Nurse – Home Health in Portland, Oregon. The ideal candidate will have prior skilled home health experience and be comfortable providing one-on-one patient care in the home setting while managing a diverse adult and geriatric patient population.

Key Responsibilities
  • Provide comprehensive home health nursing care to adult and geriatric patients.
  • Perform wound care, wound VAC management, and disease management.
  • Provide care and education for PICC lines, G-tubes, ostomies, and urinary catheters.
  • Complete accurate documentation using Epic and OASIS.
  • Conduct patient assessments and develop individualized care plans.
  • Coordinate with interdisciplinary teams to ensure continuity of care.
  • Manage approximately up to six patient visits per day while maintaining productivity standards.
  • Travel between patient homes using personal transportation.
Preferred
  • Community hospital experience.
  • Previous home health travel experience.
QualificationsRequired
  • Active Oregon RN License.
  • Active Washington RN License.
  • BLS Certification.
  • Minimum one year of skilled Home Health experience.
  • Experience with Epic charting.
  • OASIS experience.
  • Ability to provide care independently in the home setting.
  • Reliable transportation.
