Schedule: 5×8 Hour Days | 8:00 AM – 4:30 PM
Contract Length: 13 Weeks with possible extension

*Requires both an active OR and WA license*
Position OverviewSeeking an experienced Registered Nurse – Home Health in Portland, Oregon. The ideal candidate will have prior skilled home health experience and be comfortable providing one-on-one patient care in the home setting while managing a diverse adult and geriatric patient population.
Key Responsibilities- Provide comprehensive home health nursing care to adult and geriatric patients.
- Perform wound care, wound VAC management, and disease management.
- Provide care and education for PICC lines, G-tubes, ostomies, and urinary catheters.
- Complete accurate documentation using Epic and OASIS.
- Conduct patient assessments and develop individualized care plans.
- Coordinate with interdisciplinary teams to ensure continuity of care.
- Manage approximately up to six patient visits per day while maintaining productivity standards.
- Travel between patient homes using personal transportation.
- Community hospital experience.
- Previous home health travel experience.
- Active Oregon RN License.
- Active Washington RN License.
- BLS Certification.
- Minimum one year of skilled Home Health experience.
- Experience with Epic charting.
- OASIS experience.
- Ability to provide care independently in the home setting.
- Reliable transportation.
