Position Summary:
We are seeking an experienced Pharmacist with a minimum of 2 years of recent acute care hospital pharmacy experience. The ideal candidate will be responsible for medication management, IV compounding, medication reconciliation, and ensuring safe and effective pharmaceutical care. Candidates should have hands-on experience with hospital pharmacy technologies, including Allscripts, EPIC, Pyxis MedStation, DoseEdge, KitChek, and bedside barcode medication administration systems. Strong knowledge of oncology drug preparation using closed system transfer devices (CSTD), medication safety practices, and interdisciplinary collaboration is required to support optimal patient outcomes in a fast-paced acute care environment.

 

Documents Required:
  • Valid NY State Pharmacist license
  • BLS Certified through AHA
  • COVID Vaccination
Roles & Responsibilities:
  • Review, verify, and dispense medications while ensuring accuracy, safety, and compliance with hospital policies.
  • Prepare and compound sterile IV medications, including oncology treatments using closed system transfer devices (CSTD).
  • Perform medication reconciliation and collaborate with physicians, nurses, and healthcare teams to optimize patient care.
  • Utilize pharmacy technologies such as Allscripts, EPIC, Pyxis MedStation,
  • DoseEdge, and KitChek for medication management and documentation.
  • Conduct barcode medication verification, monitor medication therapy, and provide patient counseling and education as needed.
