Requirements
- Active California RN license
- BLS certification required
- Minimum 2 years of Home Health experience
- Must have worked in this specialty within the last 3 months
Responsibilities
- Provide skilled nursing care to patients in their homes
- Perform assessments and develop individualized care plans
- Coordinate care with physicians and interdisciplinary teams
- Conduct patient education and disease management
- Monitor patient progress and adjust care plans as needed
- Maintain timely and accurate documentation
Ensure compliance with home health regulations and standards
