Physical Therapist - Home Health
Assignment Duration\nThis is a 13-week contract position.
Schedule\nDay shift, Monday to Friday, 8:00 AM to 5:00 PM (5x8 hours).
Job Description\nWe are seeking a dedicated and compassionate Physical Therapist (PT) for a home health position. This role involves working with patients in their homes to improve mobility, reduce pain, and enhance the quality of life through tailored therapy interventions. The position requires a Missouri PT state license, with compact privileges possible.
Responsibilities\n- \n
- Conduct evaluations and develop individualized treatment plans. \n
- Implement therapy interventions to improve patient mobility and quality of life. \n
- Monitor progress and adjust treatment plans as necessary based on patient needs. \n
- Document patient care and progress in the electronic medical records (EMR) system (Meditech). \n
- Maintain a caseload of 8-11 patients daily, depending on travel time. \n
- \n
- At least 1 year of experience as a Physical Therapist. \n
- Missouri PT state license (PT compact privileges possible). \n
- Basic Life Support (BLS) certification required. \n
