Responsibilities

• Administer medications per physician orders and document accurately.

• Provide skilled LPN care in med-surg units, ensuring patient safety and comfort.

• Monitor patient status, report changes promptly to nursing team.

• Maintain strict adherence to regulatory and safety protocols.

• Document care activities in electronic health records with accuracy.

• Assist with patient assessments and care planning under supervision.

• Collaborate with multidisciplinary team to optimize patient outcomes.

• Demonstrate initiative in meeting targets and contributing to pipeline-based goals.

• Support professional development through ongoing learning and skill enhancement.

• Provide patient and family education on care plans and post-discharge needs.

• Participate in on-site performance reviews and data-driven quality improvements.

• Maintain a clean, organized work environment to improve efficiency.

Experience/Qualifications

• Active LPN license with current state board eligibility.

• Proven med-surg experience, with intravenous medication administration.

• Accurate medication administration and documentation in EHR systems.

• Patient safety focus with ability to monitor and report changes promptly.

• Strong teamwork and communication for multidisciplinary care.

• Compliance with regulatory standards and facility policies.
