Responsibilities
Administer medications per physician orders and document accurately.
Provide skilled LPN care in med-surg units, ensuring patient safety and comfort.
Monitor patient status, report changes promptly to nursing team.
Maintain strict adherence to regulatory and safety protocols.
Document care activities in electronic health records with accuracy.
Assist with patient assessments and care planning under supervision.
Collaborate with multidisciplinary team to optimize patient outcomes.
Demonstrate initiative in meeting targets and contributing to pipeline-based goals.
Support professional development through ongoing learning and skill enhancement.
Provide patient and family education on care plans and post-discharge needs.
Participate in on-site performance reviews and data-driven quality improvements.
Maintain a clean, organized work environment to improve efficiency.
Experience/Qualifications
Active LPN license with current state board eligibility.
Proven med-surg experience, with intravenous medication administration.
Accurate medication administration and documentation in EHR systems.
Patient safety focus with ability to monitor and report changes promptly.
Strong teamwork and communication for multidisciplinary care.
Compliance with regulatory standards and facility policies.
