Responsibilities
- Plan and conduct segments of physical therapy treatment programs designed to restore and improve physical, social, and mental functions as part of patient-centered rehabilitation.
- Co-lead interdisciplinary patient groups, coordinating care with colleagues across disciplines to support shared treatment goals.
- Deliver home-based physical therapy interventions, including therapeutic exercises, gait training, balance activities, and functional ADL training.
- Monitor patient progress, document outcomes, and adjust treatment plans to meet defined goals while ensuring patient safety and policy compliance.
- Collaborate with the interdisciplinary team to support discharge planning and transitions of care; maintain accurate and timely medical records.

Requirements
- Active Physical Therapist license in California.
- 1+ year of physical therapy experience; home health experience preferred.
- Strong communication and collaboration skills for interdisciplinary care.
- Ability to work in a daytime, 8-hour shift and participate in a 13-week travel assignment.
- Comfortable delivering therapy in a home-based setting and documenting progress in patient records.
