📍 Clearlake, CA
🕘 Day Shift | Mon–Fri | 8:00 AM – 4:30 PM
Requirements:
Active Physical Therapist License (state of practice)
Home Health experience required
HCHB & OASIS experience – Required
2+ years PT experience – Preferred
BLS / CPR (BLS, HS-BLS, or RQI BLS)
Valid Driver’s License (Must be 21+)
DPT required if licensed after Jan 2016
