Job Title: Home Health Physical Therapist
Location: Community-Based (Patient Homes & Local Service Area)
Employment Type: Full-Time / Contract
The Home Health Physical Therapist provides skilled physical therapy services to patients in their homes, focusing on restoring function, improving mobility, relieving pain, and preventing or limiting permanent physical disabilities. This role emphasizes personalized, patient-centered care while supporting individuals in maintaining independence within their community.
Key ResponsibilitiesPerform comprehensive physical therapy evaluations in patients’ homes to assess mobility, strength, balance, coordination, and functional abilities
Develop and implement individualized treatment plans based on patient needs, physician orders, and clinical findings
Provide therapeutic exercises, manual therapy, gait training, and balance interventions
Educate patients, families, and caregivers on home exercise programs, safety strategies, and fall prevention
Monitor patient progress and adjust treatment plans as needed to achieve optimal outcomes
Document all visits, evaluations, and progress notes accurately and in compliance with regulatory and agency standards
Collaborate with physicians, nurses, occupational therapists, and other healthcare professionals to ensure coordinated care
Assess home environments and recommend modifications or adaptive equipment to improve safety and independence
Participate in case conferences and interdisciplinary team meetings
Doctor of Physical Therapy (DPT) or Master’s degree in Physical Therapy from an accredited program
Current state licensure as a Physical Therapist
Minimum of 1 year of clinical experience (home health experience preferred)
Valid driver’s license and reliable transportation
Current CPR certification
Strong clinical assessment and critical thinking skills
Excellent communication and interpersonal abilities
Ability to work independently and manage time effectively
Compassionate, patient-focused approach to care
Knowledge of home health regulations and documentation standards
Adaptability to diverse home environments and patient conditions
Travel to patient homes within the assigned service area
