Position Overview:
Seeking an experienced Home Health Physical Therapist to provide one-on-one skilled therapy services to patients throughout the Medford, Oregon area. Candidates must have prior skilled home health experience and be proficient with OASIS documentation.

Required Qualifications

• Minimum 1 Year of Skilled Home Health Physical Therapy Experience
• OASIS Experience Required
• BCLS Certification
• Valid Physical Therapist License
• Own Reliable Transportation Required
• Primary COVID-19 Vaccination Series Plus One Booster Required

Experience Requirements

• Skilled Home Health Experience: Required
• OASIS Documentation: Required
• First-Time Travelers: Not Accepted

Key Responsibilities

• Perform comprehensive physical therapy evaluations
• Develop and implement individualized treatment plans
• Provide skilled therapy interventions in patients' homes
• Complete OASIS assessments and documentation accurately
• Educate patients and caregivers on treatment plans and home exercise programs
• Coordinate care with physicians, nurses, and interdisciplinary team members
• Maintain timely and accurate clinical documentation in EPIC/OASIS
