A home care agency is seeking a Coordination Director to lead and manage the Coordination Department. This role ensures the effective scheduling and coordination of home care services, oversees a team of coordinators, and collaborates with clinical and administrative departments to maintain compliance, improve client satisfaction, and support agency growth.
Responsibilities for the Coordination Director:
  • Lead and manage the coordination team, including training, supervising, and evaluating service coordinators.

  • Develop and enforce departmental policies, procedures, and workflows to ensure consistent quality and efficiency.

  • Provide ongoing support and mentorship to team members to promote a productive and motivated work environment.

  • Ensure all client cases are staffed promptly and appropriately based on client needs, caregiver qualifications, and agency standards.
  • Monitor daily schedules, service hours, and attendance to ensure continuity of care.
Qualifications for Coordination Director:
  • Minimum 3–5 years of experience in home care coordination, with at least 2 years in a supervisory or management role.
  • Strong knowledge of home care programs such as PCA and LHCSA.
  • Exceptional organizational, communication, and leadership skills.
