Job Summary: The Lead LVN will oversee daily operations and provide direct patient care in a clinic setting. This role includes supervising other LVNs, coordinating patient care, and ensuring high standards of clinical practice under the direction of an RN or physician.

Key Responsibilities:

  • Supervise and support LVN staff, ensuring adherence to clinical protocols and procedures.

  • Coordinate patient care activities, including scheduling and workflow management.

  • Administer medications and treatments as prescribed.

  • Conduct patient assessments and assist with diagnostic tests and clinical procedures.

  • Provide patient education and support regarding treatment plans and health maintenance.

  • Maintain accurate and timely medical records.

  • Ensure a clean, safe, and efficient clinical environment.

  • Assist in developing and implementing clinic policies and procedures.

  • Address patient concerns and ensure high levels of patient satisfaction.

  • Maintain clinic supplies and coordinate nursing department projects.

  • Act as a liaison between LVNs and Nursing Management to facilitate effective communication and collaboration.

  • Train and mentor LVN staff to ensure competency and professional development.

  • Practice nursing care in compliance with HIPAA standards

  • Willnes to float through out different sites as needed.

  • Assist in emergency situations as needed.

  • Perform other duties as assigned.

Required Qualifications:

  • Graduate of an accredited LVN program.

  • Current California LVN license in good standing.

  • Minimum 3 years of experience as an LVN working with geriatric or frail populations.

  • Valid CA Drivers license

  • BLS Certification from the American Heart Association

  • Bilingual in Spanish, Cantonese, or any other language is highly preferred

  • Proficiency in electronic health records (EHR) systems.

Setting: Out Patient Clinic

Population: Geriatric population

Monday - Friday NO WEEKENDS
